Who This Is For

For Problems That Keep Coming Back

This request form is for people who want to be evaluated for a recurring or unresolved musculoskeletal problem.

  • Recurring pain, tightness, or stiffness 
  • Limited movement or restricted range of motion 
  • Numbness, tingling, or nerve-like symptoms 
  • Symptoms that return after stretching, massage, chiropractic care, physical therapy, injections, or exercise 
  • Problems affecting work, sleep, exercise, golf, pickleball, lifting, walking, sitting, or daily activity

What Happens First

We Review Before We Recommend

Most new patients begin by submitting the secure New Patient Evaluation Request form.


We review what you share to help determine whether a consultation or examination may be appropriate. If your case does not appear to be a good fit for our office, we will tell you.


This does not replace a consultation or examination, and submitting the form does not create a doctor-patient relationship.

Three Questions Help Us Understand the Problem

The form is designed to collect enough information to help us review your request without making you complete a full intake before we know the appropriate next step. 

What is the biggest challenge you are trying to solve right now?

Include where the symptoms are located, how long this has been going on, and what symptoms you notice. 

What have you already tried?

Include treatment, stretching, exercise, massage, chiropractic care, physical therapy, injections, imaging, or medical visits that may be relevant. 

Why is now the right time to address this?

 Tell us what has changed or what the problem is now limiting. 

Fees and Insurance

Know the Basics Before You Request Care

Discover Soft Tissue + Spine is primarily a self-pay musculoskeletal clinic. Payment is due at the time of service.


We participate with Original Medicare Part B when Medicare coverage requirements apply. For many evaluation and soft tissue services, patients should expect to pay directly at the time of care.


Before care begins, we will explain the fees that apply so there are no surprises. Please review the full Fees & Insurance page before scheduling if cost or coverage is a concern.

Current Standard Fees

Consultation: $50
Comprehensive Examination: $250
Treatment Visit: $95 flat fee per visit
